Insulin-dependent Diabetes History

  1. Establish type and duration
    1. Date of onset, manner of diagnosis
  2. Glucose control
    1. Method and frequency of monitoring, who?
    2. Normal levels, HbA1c
    3. Hypoglycemia? Frequency and severity (adrenergic vs neuro symptoms)
    4. Diet (# and spacing of meals) calories
    5. Rx (OHA, insulin – dose, timing)
    6. Height and weight
  3. Complications and Prevention
    1. Neuropathy – ANS (impotence, constipation, diarrhea, gastroparesis, orthostatic hTN); Peripheral (anesthetic, paresthetic, hyperesthetic)
    2. Retinopathy – date of last exam, by who? And any findings?
    3. Nephropathy – known renal disease or proteinuria? Date of last U/A
    4. CVS – cardiac and PVD histories
    5. Foot care – exams, shoes, podiatr
    6. Hospitalizations for poor control? DKA, hyperosmolar nonketotic coma, hypoglycemia
  4. Risks – HTN, smoking, dyslipidemia, FHx
  5. Family history – sibling or parents with NIDDM?  Increases risk by 10-15%
  6. Past medical history
  7. PSHx
  8. Allergies
  9. Medications
  10. Social Hx
